ONAGONO QUEST EVE

In the kingdom of Anailezd, only women born to the royal family are allowed to become heroes, so the sole legitimate heir has been raised as a boy who has to pass as a girl. When the dark army Ryuko invades, that disguise becomes a job title: you're the Princess Hero now, whether you like it or not.

The story follows this reluctant hero through castles, towns, towers, and dungeons in a deliberately old-fashioned JRPG structure, fighting to push back the invasion while sorting out some very personal questions about identity along the way. Along for the trip are Aei, a blunt but good-hearted beast-girl, and Noran, a noble who plays dumb better than he actually is; both relationships deepen over the course of the journey, sex included.

Combat runs on turn-based fights against bosses and regular enemies, with fourteen costumes that swap in real time during battles and exploration. Dressing more provocatively raises an embarrassment meter and changes how townspeople react to you, which folds the game's gender-bending premise directly into its systems rather than leaving it as backstory. There are seventeen H-scenes built from fifteen base CGs, and a full clear runs around fifteen hours, closer to a real RPG than a quick detour with sex scenes attached.
